Its execution period was from Jan., 2016 to Dec., 2018. Five expected outcomes of the project were: 1. To establish Health, Medicine and Welfare Committee for the elderly care, 2. To make a plan of home-care services and its implementation by nurses and health volunteers, 3. To reform Community Health Committee for organizing systematic community activities, 4. To improve health promotion exercises at local temples targeting the elderly people, 5. To encourage positive contribution to home-care services and community activities by nurses, physical therapists and health volunteers who were trained in Saku city. Outline of the project and activities from Jan. 2016 to Sep.31, 2017 were reported in Saku University Journal 11-1.（2019）.
