| 内容記述 | 介護老人保健施設の看護・介護職641 名に自記式質問紙調査を行い、認知症高齢者の尿意をどのように判断しているのか、おむつの使用についてどのように考えているのか、そしてそれらと回答者の属性との関連を検討した。その結果、認知症高齢者が尿意を何らかのサインで表す場合は6 割以上の者が「尿意がある」と回答し、「ソワソワしている」や「ズボンを触る」等は尿意のサインと解釈されていることがわかった。認知症高齢者が尿意のサインを表出できないと「尿意がない」と判断する回答が増えた。夜間のおむつ交換では睡眠を優先し、交換回数を減らすことを良いとする者が半数以上であった。認知症ケアの経験年数の長い者は、認知症高齢者をより深く観察し、尿意を慎重に判断し、不必要なおむつ使用は避ける傾向が見られた。看護・介護者は認知症ケア経験の共有と蓄積を図り、認知症および尿失禁の病態・治療に関する系統的な学習が必要と考える。 | |||||||||||||||||||||||

| 内容記述 | This study surveyed 641 nurses and caregivers at a long-term care facility for the elderly via a selfadministered questionnaire. This study then examined relationship between the criteria that were used to determine whether an elderly individual with dementia has the urge to void and views on the use of diapers and the characteristics of the respondents. When an elderly individual displayed some sign of an urge to void, over 60% of respondents answered that the individual “has the urge to void,” and actions such as “fi dgeting” or “grabbing at one’s pants” were interpreted as signs of an urge to void. When the individual was incapable of displaying signs of an urge to void, an increasing number of respondents answered that the individual“ does not have the urge to void.” In addition, more than half of the respondents indicated that they valued sleep and that they would like to reduce the frequency of nighttime diaper changes. Respondents who had years of experience working with the elderly with dementia watched those individuals more closely and they carefully decided if an individual had the urge to void. Those respondents tended to avoid unnecessary diaper use. These fi ndings indicate that staff members who work with the elderly with dementia should share their experiences and gain further experience caring for elderly individuals with dementia. Staff members need to systematically study the pathology of dementia and urinary incontinence and the treatment of incontinence. |
